Annex 3. LIST OF TECHNICAL DOCUMENTATION DURING OPERATION OF EQUIPMENT, INSTALLATIONS AND FACILITIES OF INCREASED HAZARD

This List defines the composition of the technical documentation on labor protection required for the operation of equipment, installations and structures of increased danger. The list is compiled on the basis of the requirements of the Rules of Glavgosenergonadzor and Gosgortekhnadzor of Russia.

1. List of documentation required for the operation of electrical installations.

1.1. The owner of electrical installations approved for operation is obliged to ensure the availability, organize storage in the technical archive and maintain technical documentation in the following composition:

1.1.1. The general plan of the site with the structures applied on it and underground electrical communications.

1.1.2. Approved project documentation (drawings, explanatory notes, etc.) with all subsequent changes.

1.1.3. Acts of acceptance of hidden works.

1.1.4. Acts of testing and adjustment of equipment.

1.1.5. Acts of acceptance of electrical equipment into operation.

1.1.6. Executive working diagrams of primary and secondary electrical connections.

1.1.7. Technical passports of the main electrical equipment.

1.1.8. Instructions for the maintenance of electrical installations, as well as production instructions for each workplace or profession.

1.1.9. Passport cards or magazines with an inventory of electrical equipment and protective equipment and indicating their technical data, as well as the inventory numbers assigned to them (protocols and acts of testing, repair and revision of equipment should be attached to passport cards or magazines).

1.1.10. Drawings of electrical equipment, electrical installations and structures, sets of drawings of spare parts, as-built drawings of air and cable routes and cable magazines.

1.1.11. Drawings of underground cable routes and grounding devices with references to buildings and permanent structures, as well as indicating the installation locations of couplings and intersections with other communications.

1.1.12. General power supply schemes drawn up for the enterprise as a whole and for individual workshops and sections.

1.1.13. A set of operating instructions for the maintenance of electrical installations of a workshop, site.

1.2. All changes in electrical installations made during operation must be reflected in the diagrams and drawings immediately and signed by the person responsible for the electrical facilities, indicating his position and the date the changes were made.

Information about changes in the schemes should be communicated to all employees (with an entry in the operational log), for whom knowledge of these schemes is mandatory.

1.3. A set of necessary power supply schemes must be kept at the workplace by the person responsible for electrical facilities.

1.4. A set of operational diagrams of electrical installations of a given workshop, section or electrically related other workshops, sections must be kept by the duty officer of the workshop, section.

1.5. The main diagrams should be hung out in a conspicuous place in the room of this electrical installation.

1.6. All workplaces must have the necessary operating instructions drawn up in accordance with the requirements of the Rules for the operation of electrical installations of consumers and the Safety Rules for the operation of electrical installations of consumers, standard instructions and directive materials, taking into account local conditions, signed by the person responsible for electrical facilities and approved by the chief engineer ( technical director) of the organization.

1.7. In cases of special operating conditions for electrical installations, operating instructions should be developed for servicing these electrical installations, taking into account the nature of the work performed under these conditions, equipment features, technology, etc. and approved by the chief engineer (technical director) of the organization.

1.8. The production instructions for electrical personnel should indicate:

1.8.1. A list of instructions for servicing equipment and directive documents, diagrams and devices of electrical equipment, knowledge of which is mandatory for a manager, specialist, employee holding this position.

1.8.2. Rights, duties and responsibilities of employees.

1.8.3. Relationships with superiors, subordinates and other related personnel.

1.9. In the event of a change in the state or operating conditions of electrical equipment, appropriate additions should be made to the instructions, which (with an entry in the operational log) should be reported to employees for whom knowledge of these instructions is mandatory.

The instructions must be reviewed at least once every 3 years.

1.10. At each production site, in the workshop, there must be a set of necessary instructions according to the approved list. A complete set of instructions must be kept by the power engineer (senior electrician) - the person responsible for the electrical facilities of the workshop, site, and the necessary set - by the relevant workers at the workplace.

1.11. At substations, in the switchgear or in the premises allocated for servicing electrical installations of employees (or at the workplace of the person responsible for the electrical facilities), the following operational documentation should be located:

1.11.1. Operational scheme or layout diagram.

1.11.2. Operational log.

1.11.3. Forms of work permits for the performance of work in electrical installations.

1.11.4. Switching sheets.

1.11.5. Journal or card file of defects and malfunctions on electrical equipment.

1.11.6. Sheets of indications of instrumentation and electric meters.

1.11.7. Journal of checking the knowledge of personnel of labor protection instructions.

1.11.8. Production briefing log.

1.11.9. Journal of accounting for emergency training.

1.11.10. Lists: persons entitled to sole inspection of electrical installations; persons entitled to issue operational orders, etc.;

responsible duty officers of a higher power supply organization.

1.12. The person responsible for the electrical facilities, by his order, must establish a list of employees who can be appointed as responsible managers and foremen of work on the basis of work permits and orders, as well as those who supervise during the performance of these works.

1.13. Operational documentation within the time limits established at the enterprise (at least once a month) should be reviewed by superior electrical or administrative and technical personnel, who are obliged to take measures to eliminate the identified violations.

2. The list of documentation required for the operation of gas facilities, and the availability, storage, circulation and maintenance of which should be organized in the organization:

2.1. Regulations on the gas service of the organization, which determines the organization of work for the safe operation of the gas facilities.

2.2. Regulations on the procedure for admitting workers to maintenance and repair of gas facilities, establishing the procedure for admitting and the circle of workers admitted to performing these works.

2.3. Orders on the appointment of responsible employees in the organization and in individual workshops with specific instructions for gas equipment for which one or another employee is responsible.

2.4. Orders to determine the circle of managers and specialists entitled to issue work permits for gas hazardous work, as well as persons admitted to the management and performance of these works.

2.5. Job descriptions for managers and specialists and production instructions for workers servicing and repairing gas equipment.

2.6. The executive plan (scheme) of the underground gas pipeline, the scheme of welded joints of the gas pipeline, the plan with the application of shut-off valves, control pipes, control and measuring points.

2.7. Route maps with the drawing of wells of all underground structures associated with gas pipelines, collectors, basements located at a distance of up to 50 m on both sides of the gas pipelines.

2.8. Passports for underground gas pipelines, hydraulic fracturing (GRU); manufacturer's passports for liquefied gas tanks, pressure regulators, safety or shut-off valves, shut-off valves, gas burners, automatic control and safety of boilers, furnaces and other units. Data on repairs of gas pipelines and equipment during their operation should be entered in the passports.

2.9. Plans and schedules for the maintenance and repair of gas pipelines and gas equipment, indicating the timing of the work.

2.10. Conclusion on the study of the presence and influence on gas pipelines of stray currents, a project for a device for protecting gas pipelines from destruction by electrochemical corrosion, an act of acceptance and adjustment of electrical protection.

2.11. Plan for the localization and elimination of possible accidents during the operation of the gas facilities.

2.12. Accounting logs:

2.14. List of phone numbers of services and officials associated with the operation of the gas facilities and the use of gas in the organization, as well as gas supply organizations, emergency dispatch services of gas facilities organizations and local bodies of state technical supervision.

2.15. Protocols for testing the knowledge of personnel associated with the maintenance and repair of gas facilities and the performance of gas hazardous work.

Note. The certificate for admission to gas-hazardous work in the performance of official duties, employees must have in their hands and present at the request of the person responsible for safety in the gas sector, the inspector of the state technical supervision body.

3. List of documentation required for the operation of steam and hot water boilers:

3.1. Passports of manufacturers for boilers, superheaters, economizers with copies of certificates of compliance with the requirements of the Rules for the Design and Safe Operation of Steam and Hot Water Boilers.

3.2. Manufacturers' instructions for the installation and operation of boilers and their main units.

3.3. Certificates of the quality of their installation.

3.4. Drawings (plans, sections) for boiler plants.

3.5. Certificates of registration of boiler plants in the bodies of state technical supervision and permission for their operation.

3.6. Acts of technical examinations of boiler installations.

3.7. Magazines on water treatment (water chemistry).

3.8. Instructions for maintaining the water-chemical regime.

4. List of documentation required for the operation of stationary single compressor units or a group of homogeneous compressor units, air ducts and gas pipelines:

4.1. Diagrams of pipelines (compressed air or gas, water, oil) indicating the installation locations of valves, valves, moisture-oil separators, intermediate and end coolers, air collectors, instrumentation, as well as diagrams of electrical cables, automation, etc.

4.2. Instructions for the safe maintenance of compressor units.

4.3. Compressor log.

    General provisions

1.1. To service a hot water boiler, persons are allowed not younger than 18 years old, who have undergone special training, a medical commission, who have a certificate for the right to service boilers.

1.2. Re-inspection of the boiler room personnel is carried out at least once every 12 months.

1.3. The boiler operator must:

Re-instructing on labor safety at the workplace at least every three months;

Pass a knowledge test on the rules for the design and safe operation of steam and hot water boilers, steam and hot water pipelines.

Pass a medical examination;

Perform only the work that is included in his duties;

1.4. The operator must know:

Rules for the design and safe operation of steam and

hot water boilers;

Arrangement of the internal and external gas pipeline of the boiler room, auxiliary equipment, alarm systems and automatic control;

The effect on a person of dangerous and harmful factors that arise during work;

Requirements for industrial sanitation, electrical safety, fire safety;

1.5. During operation, the operator may be exposed to the following hazardous production factors:

High pressure and temperature of heating surfaces;

High voltage in the electrical network;

Increased levels of noise and vibration;

1.6. The boiler room operator must use the following PPE:

Suit "Mechanizer-L" or Overalls for protection against general industrial pollution and mechanical impacts;

Yuft boots with oil and petrol resistant soles;

Polymer coated gloves;

Goggles closed;

Anti-noise headphones or ear plugs;

Jacket on the warming sole;

Yuft boots insulated with oil- and frost-resistant soles.

1.7. Upon entering the duty, the personnel is obliged to familiarize themselves with the entries in the log, check the serviceability of the equipment, the serviceability of the lighting and the telephone.

1.8. After accepting the shift, the operator must check the readings of pressure gauge 9 (located on the gas pipeline in front of the burner), the readings must correspond to 18-21 kPa.

1.9. Acceptance and surrender of duty must be documented by the operator with an entry in the shift log indicating the results of checking the boilers and related equipment (pressure gauges, safety valves, automation equipment). In the shift log at 9.00, the operator must record the readings of pressure gauges and thermometers of the supply and return pipelines, pressure gauge and thermometer on the boiler, take readings of the heat and gas meter, water flow.

1.10. Unauthorized persons access to the boiler room is allowed by the head.

1.11. The boiler room, boilers and all equipment, passages must be kept in good condition and proper cleanliness.

1.12. Doors for exiting the boiler room should open easily to the outside.

1.14. It is not allowed to accept and hand over the shift during the liquidation of the accident.

    Gas preparation and start-up

      Check that the boiler is filled with water up to an operating pressure of 2.5 bar.

      Check the condition of the shut-off valves on the supply and return pipelines, they must be in the open position.

      Check the performance of the network pump No. 2

      Check the presence of gas in the SHRP at the outlet, the readings on the pressure gauge 9 should correspond to 21 kPa

      Open smoothly the ball valve on the gas pipeline after the ShRP (at the entrance to the boiler room).

      Open ball valve No. 4 on the downstream and after the counter No. 7

      Blow out the gas pipeline (ball valve No. 10 must be in the open position) for 10 minutes.

      Close the purge plug (ball valve no. 10).

      Make sure that there is no gas leakage from gas pipelines, gas equipment and fittings by washing them.

      If there are no leaks, open ball valve no. 12 (before the burner).

    Boiler operation

3.1. During duty, the boiler room personnel must monitor the health of the boiler and all boiler room equipment. Record temperature and pressure readings on the boiler, as well as pressure and temperature on the supply and return of the heating network, in a shift log. Faults detected during the operation of the equipment should be recorded in a shift log. Personnel must take corrective action. 4. Emergency stop of the boiler

4.1. In case of emergency shutdown of the boiler, it is necessary:

4.1.1. Switch off the boiler mains switch E on the Vitotronic 100

4.1.2. Switch off the boiler mains switch G on the Vitotronic 333

4.1.3. Turn off the burner on toggle switch #1

4.1.4. Shut off the gas supply to the boiler room, open the purge candle (Close the taps on the burners and the gas pipeline)

    Boiler shutdown

5.1. Stopping the gas boiler is carried out by the master of the boiler room.

    Final provisions:

7.1. The administration of the enterprise should not give instructions to the personnel that are contrary to the instructions and may lead to an accident or an accident.

7.2. Workers are liable for violation of the instructions relating to the work they perform in the manner prescribed by the internal labor regulations and in accordance with the criminal code of the Russian Federation.

For each boiler house, the owner must keep design and executive documentation, including: a design assignment and a gas supply project, in which all the changes that have taken place during the installation work and during operation have been made, or the relevant documents having the force of the project have been added (permission letters, references to other design decisions or normals). The executive drawings of the gas supply system must be accompanied by copies of drawings from other sections of the project for this boiler house (chimney, prefabricated gas ducts, ventilation systems, etc.), which are referenced in the gas supply project.

The as-built documentation should contain: acts of acceptance into operation of the thermal mechanical equipment of the boiler room, automation systems and acts of their commissioning; axonometric diagram of gas pipelines, drawings of gas and air paths, supply and exhaust ventilation; passports or forms for all installed equipment; certificates for fittings and pipes; passports for smoke exhausters and draft fans, gas burners, economizers, automatic equipment for hydraulic fracturing (GRU), measuring instruments; passport for supply and exhaust ventilation devices; construction passports for gas pipelines, hydraulic fracturing (GRU); a plan of the route of the external gas pipeline indicating wells, channels, cellars within a radius of 50 m from the underground gas pipeline; act of final acceptance of gas equipment into operation; acts of checking the ground loop, insulating flanges; passport (certificate or certificate of verification) of a gas indicator, gas analyzer.

For steam boilers, superheaters, economizers with an excess pressure of more than 0.7 kgf / cm 2 and hot water boilers with a water temperature of more than 115 ° C, subject to registration with the Gosgortekhnadzor, the boiler room must have passports that indicate: pressure and temperature of the coolant, year of manufacture, date of commissioning and other data. Boiler rooms should have: production instructions for servicing boilers, gas and auxiliary equipment, automation systems; extract from the order on the appointment of a person responsible for the gas facilities of the boiler house; a list of operators and other employees of the boiler house, indicating the date of passing the exam in safe working methods, signed by the head of the enterprise and sealed; work schedule of the boiler room staff; schedules of preventive inspections and repairs of equipment.

Operational documentation includes a list of hourly records of controlled parameters, as well as logs: shift (watch); ongoing maintenance, scheduled inspections and repairs of gas equipment; checks of safety automation and instrumentation; orders of the head of the boiler room (head of the workshop, section); training in safe working methods; registration of orders for gas hazardous work; bypassing the route of the underground gas pipeline; checking the operation of insulating flanges; conducting training sessions; orders of officials of regulatory bodies.

The shift log is designed to record in chronological order the switching and other work performed during the shift, periodic operations for checking, inspecting and testing equipment. The operator must make entries in the daily record of all the main parameters of the boiler operation according to the readings of the measuring instruments. Defect and malfunction logs are used to record the shift supervisors of problems noticed during operation, the elimination of which requires the participation of repair personnel. When recording, the equipment, device or device requiring corrections, the nature of the defect and the necessary measures to eliminate it are indicated. Defects and malfunctions eliminated by shift personnel are also recorded. In the journal of orders for the boiler room, the head of the boiler room or the person replacing him writes down operational orders with a validity period of more than a day. If the order concerns the duty personnel of all shifts, then the elders of all shifts must sign. In large boiler houses, the results of determinations of the composition of exhaust gases, performed at least once a week, are recorded in the suction control log. According to the composition of gases, the coefficient of excess air in various places of the gas path and the magnitude of suction in its individual sections are determined. Simultaneously with the determination of suction cups, the condition of the lining and insulation is checked. In the register of instructions of officials of regulatory authorities, surveys carried out by representatives of the Gosgortekhnadzor, Gosgaznadzor, gas sales organizations, etc. are noted. If, as a result of the survey, the consumer is presented with an act of instruction, then this is noted in the journal. One of the leading employees of the institution (the head of the center for labor adaptation of convicts, the chief engineer) should be responsible for registering boilers and water heaters when they are put into operation, monitoring compliance with the Rules by the personnel of institutions and bodies of the penal system.

In order to carry out technical examination, ensure good condition and constant monitoring of the safe operation of boilers and water heaters, the administration of the institution must appoint a person responsible for good condition and safe operation. The specified person is appointed from among the engineering and technical workers (head of the workshop, section, foreman) who have the appropriate qualifications and, as a rule, a heat engineering education. In some cases, responsibility for the good condition and safe operation may be assigned to an engineering and technical worker who does not have a heat engineering education, but who has been fully trained in a special program (see Appendix 4) and who has passed the examination of the commission of a specialized energy enterprise or who has completed training in a training - course combine or institute for advanced training.

The appointment of a responsible person is formalized by an order for the institution with the number and date of the order recorded in the passport of the boiler (water heater). During the absence of a responsible person (vacation, business trip, illness), the execution of his duties should be assigned by order to another engineering and technical worker who has passed the knowledge test of the “Rules for the Construction and Safe Operation of Boilers”.

Responsible for good condition and safe operation should provide:

  • a) maintenance of boilers in good condition;
  • b) carrying out timely preventive maintenance of boilers and preparing them for technical examination;
  • c) timely elimination of identified faults;
  • d) maintenance of boilers by trained and certified personnel;
  • e) maintenance personnel - instructions, as well as periodic verification of knowledge of these instructions;
  • f) compliance with production instructions by service personnel.

Responsible for the good condition and safe operation of boilers has the right to:

  • a) remove personnel from servicing boilers who violate instructions or show unsatisfactory knowledge;
  • b) submit proposals to the management of the institution on bringing to responsibility engineering and technical workers and persons from among the service personnel who violate the rules and instructions;
  • c) submit proposals to the management of the enterprise to eliminate the causes that give rise to violations of the requirements of the rules and instructions.

The basic principles of organizing the operation of boiler houses are to ensure reliable, economical and trouble-free operation of the equipment.

For this you need:

  • entrust the maintenance of the boiler house to trained personnel and periodically improve their qualifications;
  • provide service personnel with the “Production Instructions for Maintenance of Boiler Room Equipment” and other service instructions;
  • organize constant monitoring of the operation of all boiler room equipment, create a system of technical accounting, reporting and work planning;
  • properly operate all equipment in the most economical modes. Maintain thermal insulation of hot heating surfaces and use other measures to save fuel, heat and electricity;
  • draw up and accurately carry out annual schedules for preventive maintenance and overhaul of all boiler room equipment, having the required number of spare parts, repair and auxiliary materials;
  • to constantly monitor the serviceable condition of operating equipment and detect malfunctions in a timely manner.

The following types of repairs are carried out in the boiler rooms of the production departments of the penitentiary system: service(O), current(T) and capital(TO).

For boilers and boiler-auxiliary equipment, the structure and duration of repair cycles are applied:

  • maintenance - 1 time in 6 months;
  • current repairs - 1 time in 12 months;
  • overhaul - 1 time in 3 years.

According to this duration of repair cycles in boiler houses, a scheduled preventive maintenance (PPR) schedule is drawn up for all equipment.

Most of the tasks in preparing boiler room equipment for operation are associated with the concept of the quality of a product or material during its operation or use for its intended purpose. In particular, the influence of the quality of the repair carried out affects the operating conditions and, ultimately, determines the service life of the equipment.

Among the factors affecting the technical condition of the equipment, among others, one can also include the factors of the technological group: the impact of maintenance and repair and the quality of operating materials. All of them are indirectly related to operation through maintenance and repair. The training manual edited by A. V. Paturov and E. A. Koryakin “Preparation of the boiler equipment of the units of the penal system for work in the autumn-winter period” (Kirov branch of the Academy of the Federal Penitentiary Service of Russia, 2011) discussed the repair of the main equipment of boiler rooms and the structural and repair materials used .

The administration of the institution must ensure timely repair of boilers according to the approved preventive maintenance schedule and commissioning after major repairs, modernization, reconstruction, changes in the type of fuel and water regime. Repair is carried out in accordance with the technical specifications in accordance with the requirements of the Rules for the Construction and Safe Operation of Boilers.

The boiler room must be repair log, which contains information about the implementation of repair work and about stopping the boilers for cleaning and flushing. Replacement of pipes, rivets and rolling of pipe connections with drums and chambers should be noted on the layout of pipes (rivets) in the repair log. The repair log also reflects the results of the inspection of the boiler before cleaning, indicating the thickness of the layer of scale and sludge deposits and defects corrected during the repair period.

Information about repair work that necessitates an early inspection of the boilers, as well as data on the materials and welding used during the repair, must be entered in the boiler passport.

Prior to the start of work inside the drum, chamber or header of the boiler, connected to other operating boilers by common pipelines (steam, feed, drain and drain lines, etc.), as well as before inspecting or repairing pressure elements of the boiler, the boiler must be separated from all pipelines by plugs or disconnected. Disconnected pipes should also be plugged.

When operating on gaseous fuel, the boiler must be reliably disconnected and turned off from the general gas pipeline in accordance with the boiler maintenance instructions.

On valves, gate valves and dampers, when the relevant sections of pipelines, steam pipelines, gas pipelines and gas ducts are turned off, as well as on starting devices for smoke exhausters, blowers and fuel feeders, posters should be hung out: "Do not turn on - people are working." Fusible links must be removed from starters of smoke exhausters, draft fans and fuel feeders.

The plugs used when shutting down the boiler, installed between the flanges of the pipelines, must be of adequate strength and have a protruding part (tail), which determines the presence of the supplied plug. Gaskets installed between the flanges and the plug must be without shanks.

Work in the elements of the boiler plant (inside the furnaces and drums), as well as gas ducts, air ducts and chimneys, should be carried out after ventilating them from harmful gases and checking the air for gas contamination at a temperature inside the elements of the boiler plant, gas ducts, air ducts and chimneys not higher than 306 K (33 ° C) by written permission (along with) the head of the boiler room.

The time spent inside furnaces, drums, flues, air ducts, chimneys, as well as the duration of rest is determined by the person issuing the order, depending on the conditions and nature of the work, indicating this in the line of the order "Special conditions".

When working inside furnaces, drums, gas ducts, air ducts and chimneys, it is prohibited to use kerosene and other lamps with flammable materials.

When the boiler is stopped for a long period, it must be preserved.

The boiler room must have a clock, a telephone or an audible alarm to call representatives of the administration in case of emergency.

Appendix 5 provides knowledge test tickets for responsible persons and control questions for executives related to the operation of hazardous production facilities.
